Release of Information Form
Before meeting with an employee, the first step in making a successful EAP referral is to call Frank Horton Associates at 919-850-3410 or 1-800-326-3864. We will take you through the process of appropriately referring an employee to the EAP.
On this web page, you can download the Supervisory/Management Referral Release of Information form. Managers should use this form when making a formal referral to the EAP. When employees sign the release, it allows the EAP to provide feedback to the manager about the employee’s attendance and compliance with EAP recommendations. Feedback from the EAP can be invaluable when a manger is dealing with an employee with a performance problem.
Having the release signed at the time of referral will ensure FHA is able to give you the feedback you need. Of course, employees use their own discretion about whether to sign a release, but signing one generally leads to greater likelihood of job improvement and job retention.
